We would like to thank you for taking part in our new process of completing athletic participation paperwork online. This new automated process will allow us to be more efficient in handling the forms as well as saving valuable resources.

Please select the grade level your child will be in for the 2026-2027 school year

DO NOT USE ATHLETE NICKNAMES ON FORMS

Your Child’s LEGAL NAME (as used during registration- must include hyphens if applicable)

1) Please make sure all information is accurate, as this will be used in case of emergency.

2) Parent/Guardian AND Student signatures are required in order for forms to be accepted electronically.

3) To access the required athletic participation forms, place your cursor over the “Electronic Participation Forms” tab to view the drop down menu.

4) Click on the form name and fill out the information requested. If you are unsure about the answer to a question, please answer not applicable, unknown or none.

5) To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box.

6) Once you have completed each form, you will have the opportunity to print the document.

7) You will receive a confirmation email once the documents have been reviewed by the Athletic Trainers.

8) Write N/A in any required fields that do not apply to you. All fields must have something in the box.

9) Once completed, click on the “Medical History/Physical” tab to fill out and print the physical page to take with you to the doctor.

* All athletes in 7th through 12th grade must have an annual physical examination by a medical doctor to participate in practices or games.

* Note for physicals: Students are required to use the Pre-participation Physical Examination Form provided by the UIL. NO OTHER Physical Examination Form will be accepted. A new physical exam must be given prior to each school calendar year. Physicals must be dated from a physician after May 6, 2026 & 2026-2027. This form can be accessed from the Medical History/Physical tab. You are encouraged to fill out the history portion of the form prior to printing.

Physicals and Medical History forms are not submitted online. They must be completed by a physician and returned to the Athletic Trainers or the junior high head coach.
